Brain Drain: The Mere Presence of One’s Own Smartphone Reduces Available Cognitive Capacity
Our smartphones enable—and encourage—constant connection to information, entertainment, and
each other. They put the world at our fingertips, and rarely leave our sides. Although these devices have immense potential
to improve welfare, their persistent presence may come at a cognitive cost. In this research, we test the “brain
drain” hypothesis that the mere presence of one’s own smartphone may occupy limited-capacity cognitive resources,
thereby leaving fewer resources available for other tasks and undercutting cognitive performance. Results from two
experiments indicate that even when people are successful at maintaining sustained attention—as when avoiding
the temptation to check their phones—the mere presence of these devices reduces available cognitive capacity. Moreover,
these cognitive costs are highest for those highest in smartphone dependence. We conclude by discussing the
practical implications of this smartphone-induced brain drain for consumer decision-making and consumer welfare.Marketin
Managing Isolated Wetlands After Solid Waste and Tahoe: The Case of Delaware
Perhaps as few as fifteen states have adequate protection of isolated wetlands, which were left vulnerable by the U.S. Supreme Court\u27s Solid Waste decision. This paper also examines the recent Tahoe decision and the case of Delaware to assess the impact of Solid Waste on isolated wetlands. The interim between Solid Waste in 1999 and any forthcoming legislation is the most challenging time, for this is when landowners can manifest their investment-backed expectations for recently proscribed land uses. The interim also produces uncertainty, which may lead to suboptimal landowner decisions. If state law is put in place soon, the government will minimize the possibility of costly compensation for regulatory takings and minimize the degradation of isolated wetlands. Large High Resolution Displays for Co-Located Collaborative Intelligence Analysis
Large, high-resolution vertical displays carry the potential to increase the accuracy of collaborative sensemaking, given correctly designed visual analytics tools. From an exploratory user study using a fictional intelligence analysis task, we investigated how users interact with the display to construct spatial schemas and externalize information, as well as how they establish shared and private territories. We investigated the spatial strategies of users partitioned by tool type used (document- or entity-centric). We classified the types of territorial behavior exhibited in terms of how the users interacted with the display (integrated or independent workspaces). Next, we examined how territorial behavior impacted the common ground between the pairs of users. Finally, we recommend design guidelines for building co-located collaborative visual analytics tools specifically for use on large, high-resolution vertical displays

How the Structure of Choices Influences Consumer Decisions and Experiences
This dissertation contains three papers that together demonstrate how the structure of a choice process and of the information consumers encounter can influence what they think, feel, and do. Chapter 1 investigates how the structure of a choice process—and in particular, the timing of a decision to act relative to the timing of the action—influences how individuals feel at the point of acting. We demonstrate that the emotional experience of guilt is composed of two dissociable sources: one tied to the decision to act, and one tied to the behavior itself. With this unpacking, we find that introducing a temporal gap between one’s decision to act and one’s behavior allows decision guilt to decay, which reduces the guilt at the point of acting and carries important behavioral consequences.Chapter 2 probes the structure of a purchasing interaction. We compare two ways that the purchase and quantity decisions in a purchase process can be organized. In one, customers make the purchase and quantity decisions separately; in the other, customers simultaneously indicate whether and how much to buy. We find that the simultaneous consideration of both choices changes the psychology of how people decide, and ultimately increases their willingness to purchase. In 27 lab experiments and a large field experiment, we find that this “quantity integration” can lead to substantial lift.Finally, chapter 3 reveals how the structure of one aspect of a customer interaction, a small monetary incentive, can influence what people believe and accordingly what they do. We show that customers who encounter a small surcharge for failing to perform some behavior infer that this behavior is more socially normative than those who encounter a small discount for performing it. This inference carries key consequences, changing how people feel and what they decide to do in the future. Collectively, these three chapters demonstrate the critical importance of the structuring of choices and information for consumers’ beliefs, emotional responses, decisions, and behaviors

CD40L-Tri, a novel formulation of recombinant human CD40L that effectively activates B cells
CD40L has a well-established role in enhancing the immunostimulatory capacity of normal and malignant B cells, but a formulation suitable for clinical use has not been widely available. Like other TNF family members, in vivo and in vitro activity of CD40L requires a homotrimeric configuration, and growing evidence suggests that bioactivity depends on higher-order clustering of CD40. We generated a novel formulation of human recombinant CD40L (CD40L-Tri) in which the CD40L extracellular domain and a trimerization motif are connected by a long flexible peptide linker. We demonstrate that CD40L-Tri significantly expands normal CD19+ B cells by over 20- to 30-fold over 14 days and induces B cells to become highly immunostimulatory antigen-presenting cells (APCs). Consistent with these results, CD40L-Tri-activated B cells could effectively stimulate antigen-specific T responses (against the influenza M1 peptide) from normal volunteers. In addition, CD40L-Tri could induce malignant B cells to become effective APCs, such that tumor-directed immune responses could be probed. Together, our studies demonstrate the potent immune-stimulatory effects of CD40L-Tri on B cells that enable their expansion of antigen-specific human T cells. The potent bioactivity of CD40L-Tri is related to its ability to self-multimerize, which may be facilitated by its long peptide linker. Causes of death and associated conditions (Codac): a utilitarian approach to the classification of perinatal deaths.
A carefully classified dataset of perinatal mortality will retain the most significant information on the causes of death. Such information is needed for health care policy development, surveillance and international comparisons, clinical services and research. For comparability purposes, we propose a classification system that could serve all these needs, and be applicable in both developing and developed countries. It is developed to adhere to basic concepts of underlying cause in the International Classification of Diseases (ICD), although gaps in ICD prevent classification of perinatal deaths solely on existing ICD codes.We tested the Causes of Death and Associated Conditions (Codac) classification for perinatal deaths in seven populations, including two developing country settings. We identified areas of potential improvements in the ability to retain existing information, ease of use and inter-rater agreement. After revisions to address these issues we propose Version II of Codac with detailed coding instructions.The ten main categories of Codac consist of three key contributors to global perinatal mortality (intrapartum events, infections and congenital anomalies), two crucial aspects of perinatal mortality (unknown causes of death and termination of pregnancy), a clear distinction of conditions relevant only to the neonatal period and the remaining conditions are arranged in the four anatomical compartments (fetal, cord, placental and maternal).For more detail there are 94 subcategories, further specified in 577 categories in the full version. Codac is designed to accommodate both the main cause of death as well as two associated conditions. Combined rotation scarf and Akin osteotomies for hallux valgus: a patient focussed 9 year follow up of 50 patients
<p>Abstract</p> <p>Background</p> <p>The Cochrane review of hallux valgus surgery has disputed the scientific validity of hallux valgus research. Scoring systems and surrogate measures such as x-ray angles are commonly reported at just one year post operatively but these are of dubious relevance to the patient. In this study we extended the follow up to a minimum of 8 years and sought to address patient specific concerns with hallux valgus surgery. The long term follow up also allowed a comprehensive review of the complications associated with the combined rotation scarf and Akin osteotomies.</p> <p>Methods</p> <p>Between 1996 and 1999, 101 patients underwent rotation scarf and Akin osteotomies for the treatment of hallux valgus. All patients were contacted and asked to participate in this study. 50 female participants were available allowing review of 73 procedures. The average follow up was over 9 years and the average age at the time of surgery was 57. The participants were physically examined and interviewed.</p> <p>Results</p> <p>Post-operatively, in 86% of the participants there were no footwear restrictions. Stiffness of the first metatarsophalangeal joint was reported in 8% (6 feet); 10% were unhappy with the cosmetic appearance of their feet, 3 feet had hallux varus, and 2 feet had recurrent hallux valgus. There were no foot-related activity restrictions in 92% of the group. Metatarsalgia occurred in 4% (3 feet). 96% were better than before surgery and 88% were completely satisfied with their post-operative result. Hallux varus was the greatest single cause of dissatisfaction. The most common adverse event in the study was internal fixation irritation. Hallux valgus surgery is not without risk and these findings could be useful in the informed consent process.</p> <p>Conclusions</p> <p>When combined the rotation scarf and Akin osteotomies are an effective treatment for hallux valgus that achieves good long-term correction with a low incidence of recurrence, footwear restriction or metatarsalgia. The nature of the osteotomies allows early return to normal shoes and activity without the need for postoperative immobilisation in a plaster cast.</p
Genomic Profiling of a Randomized Trial of Interferon-α versus Hydroxyurea in MPN Reveals Mutation-Specific Responses
Although somatic mutations influence the pathogenesis, phenotype, and outcome of myeloproliferative neoplasms (MPNs), little is known about their impact on molecular response to cytoreductive treatment. We performed targeted next-generation sequencing (NGS) on 202 pretreatment samples obtained from patients with MPN enrolled in the DALIAH trial (A Study of Low Dose Interferon Alpha Versus Hydroxyurea in Treatment of Chronic Myeloid Neoplasms; #NCT01387763), a randomized controlled phase 3 clinical trial, and 135 samples obtained after 24 months of therapy with recombinant interferon-alpha (IFNα) or hydroxyurea. The primary aim was to evaluate the association between complete clinicohematologic response (CHR) at 24 months and molecular response through sequential assessment of 120 genes using NGS. Among JAK2-mutated patients treated with IFNα, those with CHR had a greater reduction in the JAK2 variant allele frequency (median, 0.29 to 0.07; P < .0001) compared with those not achieving CHR (median, 0.27 to 0.14; P < .0001). In contrast, the CALR variant allele frequency did not significantly decline in those achieving CHR or in those not achieving CHR. Treatment-emergent mutations in DNMT3A were observed more commonly in patients treated with IFNα compared with hydroxyurea (P = .04). Furthermore, treatment-emergent DNMT3A mutations were significantly enriched in IFNα–treated patients not attaining CHR (P = .02). A mutation in TET2, DNMT3A, or ASXL1 was significantly associated with prior stroke (age-adjusted odds ratio, 5.29; 95% confidence interval, 1.59-17.54; P = .007), as was a mutation in TET2 alone (age-adjusted odds ratio, 3.03; 95% confidence interval, 1.03-9.01; P = .044). At 24 months, we found mutation-specific response patterns to IFNα: (1) JAK2- and CALR-mutated MPN exhibited distinct molecular responses; and (2) DNMT3A-mutated clones/subclones emerged on treatment
